For painting, there are usually no extra costs unless the job involves the outside of a double storey house - which will then need scaffolding. The state of the walls or surfaces before the job starts will affect the cost if they require more patching or sanding before painting.
This is a varied one - you can supply paint yourself but some painters prefer to work with specific brands that they recommend, so remember to check with your painter before you purchase. Also, a painter will usually leave you a portion of paint for patch-ups in future even if they supply the paint.
