Replace collapsed floorboard (borer damage), inspect for further damage

A floorboard has collapsed under our dining room. I had a look under the house, where there is about 1 metre clearance (and less under some areas). It looks to me like borer damage (photos are available).
Our single-level house was built in about 1916. We renovated about 12 years ago and extended out the back. The original part of the house, comprising 2 bedrooms, lounge and dining, has timber floorboards (possibly original) and wall-to-wall carpet.
I'm looking for someone to inspect the damaged area (and ideally also the rest of the floorboards of the original part of the house), advise on what floor repairs they think we should make, and then do them. The initial inspection would have to be done from under the house. If the inspection takes more time than is reasonable for preparing a quotation, then I’m happy to pay for that excess time.
I'm assuming that we'll separately have to get a carpet layer in to lift and then later re-lay the carpet wherever floorboards must be replaced.
Note that we do have regular termite inspections done (the last one was April 2011), but these don’t look for borer damage.
