Concrete driveway car ramp to garage

I have a garage with a sizeable step up into the main entry that is too high for my car to drive in. The step is approximately 12cm high on the left side and 19 cm high on the right.
I would like to put in a concrete ramp roughly 4.6m wide, which aligns with the garage door width, and 4.3m long. As mentioned above, at its highest point it will be 19 cm high and will obviously taper down to the current ground level.
The figures I have for concrete pricing is around $130 per cubic meter.
On the assumption that a rectangular slab with a depth of 20 centimetres would be about 4 cubic meters (4.3 x 4.6 x 0.2) and that the triangle shape would effectively cut that volume in half, I'm assuming it would be about 2 cubic meters at a cost of $260 in concrete alone.
A sizeable car port extends from the garage and is already concreted. The new concrete ramp will reside on top of this.

Initially it was tough to get a confirmed time to get an initial quote, but in the end it was significantly better than the other quote I received for the same job. Once we agreed on a price, the turn around time to complete the job was only a matter of days. The job itself was done in one day and was exactly what I was after. There was a bit of a mess left behind, but nothing too serious. On completion I paid in cash and was promised a receipt via email, but this is yet to materialise. 09/08/2011: Not sure whether the response from the A Grade was just published now or whether the notification of their response was just significantly delayed, but it's been almost two years since this job completed and I just received an email informing me of the response. While I generally thought the overall job was pretty good and have commended the work to friends and family in the two years since it was done, the response to my feedback is a bit misleading. Firstly, I gave him my business card with my email address on it and he said his wife would email me the receipt. If he did drop it in the mailbox, we never found it. As described in my comment about the quote, I was very flexible. I had to reschedule numerous times to be at home when he was available after having to work around his changing schedule. To summarise, I thought this guy did a great job, but my receipt never turned up as promised and I had adjust my work day a number of times to meet his changing schedule to come over for the quote.
