Damp proofing of concrete slab and rising damp on brickwork

We have rising damp evident on the brick outer walls (however no evidence that this has affected our plaster and paint work inside the house). Also our concrete slab seems to be absorbing some water which is causing our floating floors to warp and expand. Would like and expert to provide a quote and options on how to fix this problem.
Would look for options to try and fix the concrete slab problem immediately as we are pulling up most of the affected areas of the floating floors and replacing with tiles. The brick work depending/rising damp issue will look to be fixing in the next couple of months depending on the resolution options
